CCR ANNUAL RENEWAL CHECKLIST
In order to make better use of our time during your family member’s annual meeting, we are now able to offer online renewal of many of the standard annual forms.
If you lose your connection or are unable to complete all of these forms at once, you may return to this page and continue where you left off. You do not need to re-submit any forms that were submitted successfully, simply check off the forms you have already submitted and continue with the forms you still haven’t submitted.
Note- It is highly recommended you complete these forms on a computer. You may complete them on a phone or tablet, but it will be much more difficult.
